Parent Starter Kit

All the following are documents that we require before starting care. Please read the parent handbook and give the parent handbook acknowledgment page to the provider. The contract can be filled out online or in person. The non-prescription medication form must be filled out for each child. We require up-to-date immunization records for each child. All the following documents must be given to the provider, Jennyfer R., before the first day of care.
